Welcome to Cougar Band! Every year we have to get all our information updated and forms signed. This page will give you what you need to be ready to rock and roll. Use this list to keep track:
- Complete the General Band Parental Permission Form (available on this page).
Submitting this form will grant you access to Cut Time, but it may take a couple of days to get set up. You’ll receive a text once access is ready. -
Complete the 2025–2026 Combined Agreement Form, which includes:
• Band Fees Agreement
• Band Handbook Acknowledgement
• UIL Permission Form

Complete the Meal Selection Form (optional, available on this page)

Get your student a physical exam using the Physical Exam Form (available on this page). Physicals are required for all 9th and 11th graders, even if one was completed last year. This also applies to new NISD students or anyone required to submit an annual physical. Send completed forms to justin.murphy@nisd.net with “Physical” in the subject line.

Complete the Background Check Form if you’re planning to volunteer (available on this page). A new form must be submitted each school year.

Pay your band fees in CutTime! (Once your account is set up, you’ll receive a text with a login link. From there, you’ll be able to log in and pay your band fees directly in CutTime. Please allow a few days after submitting the permission form to receive the login link.)

Important Links:
- Physical Exam Form for Doctor – Physicals are required for all 9th and 11th grade students, even if they had one last year. This also applies to students who are new to NISD and any student who must submit a physical annually. Completed forms should be emailed to Justin Murphy at justin.murphy@nisd.net with “Physical” in the subject line.
- Volunteer Background Check Form – If you plan to volunteer for any school activity or event, you must complete this form. A new background check form is required each school year.
